Sacral nerve stimulation is the most effective surgical treatment for faecal incontinence and should be considered in patients who have failed conservative treatment. It is a two-stage procedure, with an initial peripheral nerve evaluation with temporary wires into the lower spine, followed by a subsequent permanent stimulator placement if there is detectable benefit. We know that up to 1/3 of patients have a complete improvement in their symptoms, and 70-80% of patients have a >50% reduction in episodes. The results of SNS also appear durable with 90% of patients showing improvement at 5 years.
