Comprehensive Geriatric Assessment, Dementia Assessment or Cognitive Assessment, Falls, Incontinence, Nursing Home Reviews, Medication reviews to reduce the number of medications and hence the cost of medical management in people aged 65 years of age or above with multiple and complex health problems. Assessment and management of Dementia, Alzheimer's Disease, Geriatric Syndrome (delirium, falls, incontinence, mobility/balance problems, reduced functionality), Psycho-geriatrics including management of BPSD and Ortho-geriatrics. To develop Holistic (including allied health options), Optimal (the most appropriate), Personalised (as each patient has his/her own unique body system embedded in his/her own social environment) care plans for Elderly which are patient-centred by working with and in partnership with the patient, family members/carers and other key stake holders.
